Custom Lotto Number Form

I need a straightforward design for a paper based form that lets players select (X) their own lotto numbers—and nothing else. The form must:

• User will manually select their numbers by X in a number box on this paper entry form
• Each user to choose 6 numbers from 1 to 36 (6 rows of 6)
• Skip every other data field: no names, emails, or payment information will be captured here—this form focuses purely on number selection.

This is NOT a digital form >> its paper based where user will X their numbers, that will then be scanned. A clean layout with simple plain design that can be scanned to collect user selections is whats required.
